What is penoplasty?
Penoplasty, also known as phalloplasty, is the penile cosmetic surgery procedure that allows for lengthening and/or widening and is intended for men whose penises are of a relatively normal size, but are causing them physical and psychological problems.
Enlargement penoplasty
Enlargement penoplasty aims to increase the circumference of the penis, both at rest and when erect. This is the most
common procedure.
Enlargement penoplasty is performed by lipostructure or lipofilling. It consists of increasing
the diameter of the penis by injecting fat into this area. The fat cells are removed from the patient himself and
cause an increase in the volume of the penis, both at rest and when erect.
Enlargement penoplasty
Extension penoplasty (or phalloplasty) only gives results on the penis at rest. It is often performed on athletes
and has no effect on sex life.
Extension is achieved by releasing the penile retaining ligament, which
connects the cavernous tissue to the pubic bone to free the penis. The edge is pulled out and the cavernous
tissue is released. The extension varies between 2 and 5 cm but only applies to the flaccid penis.

Before the penoplasty operation
- As with any surgical procedure, the patient must undergo a pre-operative check-up following the surgeon's recommendations. A consultation with the anaesthesiologist is essential at the latest 48 hours before the operation.
- It is strongly recommended to stop smoking 15 days before and 2 months after the surgery in order to promote fat grafting for the enlargement and healing for the extension.
- No aspirin-based medication should be taken for 10 days before the operation. Preventive treatment is prescribed for patients prone to genital herpes to prevent an outbreak after the operation.
- You must fast (not eat or drink) for 6 hours before the operation.
- Treatment to temporarily suspend erections is taken to ensure proper healing.
Penis enlargement
Course of action
Atraumatic fat extraction is performed through hidden incisions in the stomach area, thighs or another area with a very fine suction cannula. The extraction areas are chosen according to the amount of fat needed and the different possibilities. The amount of fat extracted depends on the excess and the morphology of the penis. The fat cells are cleaned, centrifuged (Coleman technique) and injected through small incisions with fine cannulas in the form of microparticles between the skin surrounding the penis and the cavernous tissues in independent paths to achieve a harmonious result. Lipostructure offers a permanent result but the transplanted fat cells are sensitive to changes in the patient's weight. Weight stability is strongly recommended.
After enlargement
The penis is very swollen in the days following the operation and the result is exaggerated due to oedema. The
gradual deflation of the first few days is normal: the swelling disappears but the fat cells remain. Bruises may
appear in the areas where the fat has been extracted for about 10 days. They rarely affect physical
activities.
You can take a shower the day after the surgery. No bandages are required.
You will need to
wear loose-fitting pants for the first few weeks, as briefs may cause inappropriate compression.
Usually you
do not need to stop working.
The stitches will heal on their own after 10 to 15 days, when the healing is
complete.
It is necessary to wait 2 to 3 weeks before sexual activity, which is however possible after the
anti-erection treatment. Wearing a lubricated condom will prevent irritation.
Activities such as
motorcycling, cycling or horse riding can be resumed after 1 or 2 months.
Phalloplasty result
The final appearance will be appreciated in the 8 to 10 weeks following surgery. This is the time needed for the
swelling to disappear and the tissues to become supple again. The volume of the penis is definitive. There can
be a variable difference of up to 30% between the amount of fat injected and the fat that remains on the penis,
but the surgeon takes this variation into account when defining the volume of fat injected.
At rest, the
circumference can increase by 2 to 3 cm. The erect penis is also enlarged but the result is less visible. A new
distribution on the erect penis is also visible.
The scars gradually fade.
The cavernous tissues are not
involved, so the erection is not affected after this procedure.
The result is permanent, but the patient must
maintain a stable weight or not lose too much weight, even in the long term. The transferred fat is sensitive to
weight variations. A loss of weight would degrade the result, a gain of weight would improve it.
The
advantage of this procedure is that it preserves the natural appearance of the penis. The operation is
completely invisible, you do not have to reveal the procedure to a future partner if you do not wish to.
Penis enlargement
Course of the Penian enlargement
The release of the penile ligament is done according to the personal technical habits of the
surgeon, who will adapt his actions to you. The incision creates a scar of varying shape, such as an inverted Y
over the pubis and down to the scrotum or a Z. Sometimes, depending on the shape of the scrotum, a small
additional incision is required at the base of the penis, over the testicles. A non-compressive sealed dressing
is applied at the end of the procedure. The operation lasts between 1 hour and 1.5 hours.
The extension
obtained (2 to 5 cm) depends on the anatomical conditions and is unpredictable before the operation. The penis
is elongated at rest but the extension during erection is usually minor.
The pain is usually minor and rarely
requires the usual analgesics.
Any untimely erection will require an adjustment of the preventive treatment.
After extension
Bruising and swelling usually occur after the procedure.
Careful and gentle local care will be given for about
10 days until healing is complete.
Protective briefs will be worn over the dressing for 15 days to avoid
traction on the scar.
The stitches will heal on their own after 10 to 15 days, when the healing is
complete.
You can take showers once the healing is complete.
Sexual activity is not recommended for 3
weeks.
You will have to stop working for 5 to 10 days, depending on your profession. If you have a sedentary
occupation, you can return to work in the days following the operation. Any physical activity is forbidden until
the recovery is complete.
Activities such as motorcycling, cycling or horse riding can be resumed after 1 or
2 months.

Extension result
The result will be assessed 15 days after the operation, when the skin scar is present. The increase is between 2
and 5 cm at rest and is non-existent or minor during erection.
After surgery, the scars are pinkish and
inflammatory. They gradually fade.
Sometimes patients ask for hair removal because of pubic hair coming down
to the base of the penis.
The strength of the erections is intact, but in some cases it is a little less
vertical (by a few degrees). This does not affect sexual intercourse.
Hidden in the pubic hair, the scar will
not reveal the operation, which will remain invisible. You do not have to reveal the procedure to a future
partner if you do not wish to.
This surgery is intended to provide improvement and not to achieve dramatic
volume and length.
